Patients are Bringing AI To Therapy

The American Psychological Association's (APA) 2026 Chatbots and Mental Health Survey found that AI is becoming a common part of mental health care, but psychologists remain cautious about its role. Based on responses from more than 1,200 licensed U.S. psychologists, the survey suggests that many patients are using AI to supplement—not replace—traditional therapy.

  • AI use among patients is widespread.
    • 77% of psychologists said patients have discussed using AI.
    • 39% reported patients used AI to self-diagnose.
    • 35% said patients viewed AI as an additional mental health professional.
    • Patients also used AI for therapy support (33%), self-discipline and reminders (34%), companionship (22%), and even intimate relationships (13%).

  • Psychologists observed both benefits and risks.
    • Some patients felt validated and supported after interacting with AI.
    • AI sometimes encouraged patients to discuss difficult emotions or practice therapy skills.
    • However, psychologists also reported concerns about:
      • Dependency on chatbots (36%)
      • Unhealthy communication (25%)
      • Distorted thinking or delusional beliefs (15%)

  • Safety concerns are significant.
    • 97% of psychologists worried chatbots could reinforce harmful beliefs.
    • 94% believed current AI lacks the nuance needed to treat mental health conditions.
    • 89% were concerned AI could inadvertently encourage self-harm during crises.
    • 94% did not trust technology companies to adequately protect users' mental health data.

The APA emphasizes that AI should not replace licensed mental health professionals. Instead, AI may be useful when it:

  • Helps organize thoughts before therapy.
  • Assists with therapeutic homework or coping strategies recommended by a clinician.
  • Encourages users to consider multiple perspectives rather than simply validating their beliefs.

The APA advises users to:

  • Verify AI-generated mental health information with a healthcare professional.
  • Avoid using AI to diagnose mental health conditions.
  • Never make major life decisions based solely on AI advice.
  • Be cautious about sharing personal health information, as conversations may not be private.
  • Inform their therapist if they are using AI as part of their mental health support.

The survey suggests that AI is increasingly being incorporated into mental health support, often as a companion to therapy rather than a replacement. While AI may help users reflect, organize thoughts, and practice coping skills, psychologists overwhelmingly believe it lacks the judgment, ethical safeguards, and nuanced understanding required for independent mental health care. The APA recommends viewing AI as a supplementary tool that works best alongside care from a licensed mental health professional—not as a substitute.
